Then you will need to fill out your personal information at the top of the application, get your prospective employer to fill out the information required in the middle portion, and then have a parent or guardian sign the bottom portion of the application.

After filling out the form and ensuring all parties involved have signed. Submit the completed work permit application to the College and Career Readiness Department (2465 Dolan Way San Pablo, CA 94806), where the work permit will be processed and ready to be issued to you.
